Referees and Assistant Referees please use this form to report your Red/Yellow cards. Reports are due 60min after the final whistle. (Click the link)

CC Reports are due 6pm Central Time of the 1st day following the match. 

Teams/MLR or on behalf of the commissioner or his Designate may refer any incidents of potential foul play. Reports must be submitted by 1pm Central Time of the 1st day following the match.
